Under 18's are unable to use the online signup because a parent/carer signature is required. If applicant is under 18 years please download and complete the application form (available in PDF format), and send it with appropriate registration fee (cheques made payable to 'Ebenezer Evangelical Church') to the 'The Noise Office', c/o Ebenezer Church, 286 Filton Avenue, Horfield, Bristol, BS7 0BA. Alternatively, please phone 0117 9791399 to request a form.

Over 18's can download and complete the form and return it to the Noise office with the appropriate registration fee OR can use the ONLINE service - details below.

If applicant is over 18 and would like to pay online using a credit card (using secure PAYPAL system), then please read and fill in the form below, following the instructions carefully. You will be required to sign your form when you register on the Noise weekend. Please fill in all the information required.

LIABILITY DISCLAIMER/ CONSENT FORM

I agree to participate in voluntary assistance for The Noise, which will take place on and during 2nd-4th May 2009 in North Bristol and all locations in connection with the event as initiated by The Noise organisers.

I understand that The Noise organisers will do their utmost to ensure the safety and security of all delegates and staff members at all times during the event. I agree to comply with any reasonable request given by Noise organisers regarding Health & Safety over the weekend.

I understand that all activities carried out during this event are carried out at my own risk and accept all responsibility. I agree not to bring any claim against the organisers of The Noise, or churches involved in this event.

I understand that it may be necessary to arrange transport in individual project member's cars, as well as Noise minibuses. Child Protection policies will be adhered to in these situations.

I give my consent to be filmed and/or photographed and I authorise The Noise organisers to use such film/photographs in presentations. The Noise operates a child protection policy.

I agree to my details being kept on a database for purposes of The Noise Weekend.
